Personal
Care’s goal is to provide exceptional care and support to our clients
and their families. From the first call, we meet with you and your
family to determine the kind of help you need. Because we recognize
that care needs are specific to the person being served, a registered
nurse evaluates each new client. The RN develops a personalized
Service Plan that addresses health care needs and household tasks,
and makes recommendations about staffing needs.
Personal Care performs tasks for your loved one with the kind of
care and concern found in a family
even very private duties like grooming and bathing. We work closely with your doctor, other health care professionals and your family
and your community, to assure optimal quality care.
Personal Care provides care for every level
of need:
Certified Nursing
Assistants (CNAs)
Licensed Practical
Nurses (LPNs) and
Registered Nurses
(RNs)
Geriatric
Care Management Services
Certified Nursing Assistant
Assists
with grooming and hygiene
Monitors
vital signs and medication
Assists
with proper body movement and daily exercise
Assists
with going to appointments
Provides
security to assure safety and well being
Secures
the right help in emergencies
Prepares
meals
Performs
the day-to-day cleaning and laundry
Accompanies
on walks
Licensed Practical Nurse/Registered
Nurse
RNs and LPNs provide home care, offering similar services as in
the hospital. Doctors’ orders are required for skilled nursing shifts.
Duties include:
Assesses
blood pressure, pulse and respiratory status
Monitors
medication regime
Monitors
nutrition, hydration, bowel and bladder functions
Diabetic
teaching check glucose levels, administer insulin
Wound
care dressing changes
Catheter
care/changes-urethral and suprapubic
Colostomy/ileostomy
care
Draws
blood samples venous
IV
therapy, antibiotics therapy
Tube
feedings
Additional
services as needed
Geriatric
Care Management Services
Personal Care also offers Geriatric Care Management Services both
to families who use our nursing services and to those who do not.
The Geriatric Care Manager:
Identifies
client’s needs
Develops
a plan to appropriately address needs
Provides
education on client’s condition
Evaluates
living conditions and makes recommendations for long-term care
Identifies
community resources
Supervises
privately employed staff
Bill
paying
Grocery
shopping
Schedules
appointments
Arranges
transportation
Writes
monthly/quarterly assessments on client
